Contract

Welcome new and continuing Horn Students!
I'm very excited to get to know you all, and look forward to a year filled with lots of musical growth!

I am constantly available by phone at 520-390-7254,
text messages to the same number,
or by email to <heatherblase@gmail.com>.
Please do not hesitate to contact me for any reason!!

Please keep the following rules in mind, so your student and I can focus on learning:
The Instructor” refers to Heather Suchodolski, while “the Student” refers to both the parent and horn student.

Attendance:
  • It is the Student's responsibility to notify the Instructor if the Student may miss a lesson, including, but not limited to the following reasons:
    • Absences / potential absences due to illness or scheduling
If the Student becomes ill on the morning of his or her lesson, please call me at
520-390-7254 as soon as possible.
  • School Schedule changes (testing, pep rallies, etc)
  • School Activities (field trips, testing, band competitions or performances)
  • School Closures/Delays due to Bad Weather

  • 24 hours notice prior to a missed lesson is required in order for the Student to not be held financially responsible for the full cost of the lesson.

  • A lesson will be considered missed and the Student shall be responsible for the full cost of the lesson if the Student is:
    • Absent (due to illness, forgetting, a field trip, testing, etc) without 24 hours prior notice
    • Tardy by 15 or more minutes to a 30 minute lesson
Exceptions for last-minute emergencies and illness may be considered on a case-by-case basis.

  • The Instructor will provide written notice of the upcoming month's lessons before the first lesson of the month. This notification serves as the Student's reminder of the upcoming month's lessons.

  • The Instructor agrees to attend all lessons, and provide the maximum notice possible if a lesson must be missed. In the unanticipated event that the Instructor should cancel a lesson with less that 24-hours notice, with the exception of extreme illness, the lesson will be made up at the Student's convenience at no cost to the Student.

  • Lessons are not a substitute for adequate practice and preparation on the Student's part. The Student should practice daily (or semi-daily, if a beginner). Lessons will typically cover material outside of the Student's band class, unless a particular aspect merits instruction.

Please call/text (520-390-7254) or email me (<heatherblase@gmail.com>)
as soon as you know the Student will miss a lesson!
Even the shortest notice helps me to not worry about the safety and well-being of your student.
Materials:
  • The following materials are recommended for the Student to own:
    • 6th grade     None required. Your 6th grade band book will suffice. Additional materials may be suggested as the Student progresses.
  • 7th - 8th grade:
Edwin Miersch: Melodious Studies for French Horn
  • 9th - 12th grade:
Max P. Pottag: Preparatory Melodies to Solo Work for French Horn

G. Kopprasch: Sixty Selected Studies for French Horn, Book 1

  • Additional materials such as scale sheets, warm-up exercises, and technical exercises may be provided by the Instructor. Please consider investing in a sturdy folder to house the lesson materials.

  • The Student must attend every lesson with valve oil, slide grease, and a mechanical or sharpened pencil, including an eraser. Using ink, including highlighter, on music should be avoided at all costs.

Payment:
  • The district-set rate for private lessons is $______ per half hour.

  • Payment for lessons should be remitted at or before the first lesson of the month, for all lessons scheduled for that month. (Example: if notified at end of September of 5 lessons in October, the Student brings payment for all 5 lessons to the first lesson of October)

  • Payments may be made in one of the following ways:
    • Exact Cash, Check, or Money Order delivered or mailed to the Instructor by the Student
    • Paid via PayPal's “Send Money to Family or Friends” sent to heatherblase@gmail.com

  • If more than two (2) lessons pass unpaid, lessons may cease until the Student's balance is paid.

  • The Student is responsible for the full cost of every lesson at the start of each lesson, unless otherwise personally, previously arranged.

  • Scholarships may be available through your school's band program. It is the Student's responsibility to pursue these means of defraying the cost of lessons.

  • If problems arise regarding payment for lessons, it is the Student's responsibility to contact the Instructor by phone at 520-390-7254 or email to <heatherblase@gmail.com> to work out a plan. Flexibility on behalf of the Instructor is not entirely out of the question.

  • Discontinuing lessons requires 2 weeks notice in order to not be charged for future lessons missed.
    • If two weeks notice is not provided, an up-front payment of 2 lessons may be required.
    • Should financial reasons begin to prohibit lessons, it is the Student's responsibility to pursue the option of scholarship funds through the school's band program before discontinuing lessons.
(Side Note: Please remember: while I thoroughly enjoy teaching each Student, funds from teaching lessons are my only source of income. Remember, your on-time payment helps me to pay my rent! Thanks! – HS)
